I, as the participant, acknowledge and agree to the terms and conditions set forth for my involvement in the dog walk events, collectively referred to as the “Events.” These Events are organized and/or hosted by the City of Mount Vernon, Mount Vernon Music & Arts Festival, Order Of Eastern Star Of Ohio, The Ohio Eastern Star Home, and/or the Ohio Eastern Star Service Dogs, referred to as the “Organizers.”.

1. Assumption of Risks: I acknowledge that participation in the Event(s) involves certain inherent risks, including but not limited to accidents, injuries, illnesses, property damage, or the loss of personal belongings. I understand and accept these risks and agree to assume full responsibility for any resulting consequences.

2. Health and Vaccinations: I certify that the dog(s) I will be walking during the Event(s)
are in good health and have received all necessary vaccinations. I understand that it is my responsibility to ensure my dog(s) are fit for participation and free from any contagious diseases.

3. Control and Behavior: I understand and agree to always maintain full control of my
dog(s) during the Event(s). I will ensure that my dog(s) will be kept on a leash and properly restrained as required. I take full responsibility for any damage, injury, or loss caused by my dog(s) to other participants, their dogs, or any third party.

4. Release and Waiver of Liability: In consideration for being permitted to participate in the Event(s), I hereby release and discharge the “Organizers”, along with their staff, its affiliates, employees, volunteers, sponsors, and all other parties associated with the Event(s) from any and all claims, liabilities, damages, costs, or expenses, including but
not limited to medical expenses and legal fees, arising from my participation in the Event.

5. Indemnification: I agree to indemnify and hold harmless the “Organizers”, its affiliates, employees, volunteers, sponsors, and all other parties associated with the Event(s) from any and all claims, demands, actions, suits, or damages, including but not limited to those resulting from my own negligence, the negligence of others, or the actions or behavior of my dog(s) during the Event(s).

6. Media Release: I grant permission to the “Organizers” to use any photographs, videos, or recordings taken during the Event(s), including images of myself and my dog(s), for promotional and marketing purposes without any compensation to me.

7. Severability: If any provision of this liability waiver is deemed invalid or unenforceable, the remaining provisions shall continue in full force and effect.
